Notation by Fownd is a real-time AI clinical documentation assistant built specifically for physical therapists. Unlike traditional AI scribes that simply transcribe conversations, Notation captures clinical reasoning and builds structured, compliant documentation during care delivery. Notes are generated in real time, aligned to SOAP format, and designed to reduce after-hours charting. Notation is built by PTs for PTs, ensuring documentation reflects true clinical flow rather than forcing rigid templates. It supports outpatient, home health, and multi-location clinics while prioritizing privacy, security, and compliance. By shifting documentation into the treatment session itself, Notation helps clinicians reclaim time, reduce burnout, and focus more fully on patient care. Visit https://fownd.care/ for more
